Avec des sourcils relevés et des pupilles bleues qui brillent sur des orbites noires, un fantôme clair s'enroule autour du côté de sa tête avec des marques de larmes coulant sur son visage, tandis qu'un haut-de-forme noir repose sur la tête de Freddy. L’extérieur de son emballage montre un fantôme s’échappant dans un long couloir, tandis que l’intérieur montre le même fantôme volant à travers la bouche d’aération de la salle de sécurité.
